If you have issues uploading a photo, this may be because you are using a cellphone. Apple iPhones save photos in an .HEIC format, which this form cannot read. If you have this issue, try saving the photo to your computer and try uploading the photo again. .jpg and .png are the only acceptable file formats for photo upload.

The photo file size cannot exceed 10.6MB.
